Privacy Policy
How Federal Injury Specialists collects, uses and protects information submitted through this website. Effective date: September 19, 2026.
1. Who we are
This website, federalinjuryspecialists.com (the "Site"), is operated by Federal Injury Specialists ("we," "us," "our"), a medical practice located at 531 West 235th Street, Bronx, NY 10463. You can reach us at (718) 432-1323 or federalinjuryspecialists@gmail.com.
2. This policy covers the website, not your medical record
This Privacy Policy explains how we handle information you provide through this Site — for example, when you submit an appointment request form or call a number listed here. Information you provide once you become a patient (your medical history, treatment records, OWCP claim documents and similar protected health information) is governed by our Notice of Privacy Practices under the Health Insurance Portability and Accountability Act (HIPAA), which is provided at your first visit and available on request. Where this policy and the Notice of Privacy Practices differ regarding protected health information, the Notice of Privacy Practices controls.
3. Information we collect
Information you give us
When you submit our appointment request form or contact us, we collect the information you choose to provide, which may include your name, email address, mobile phone number, preferred appointment day and time, the type of appointment you are requesting, your federal agency or employer, and a description of your injury or reason for the visit. Please do not include Social Security numbers, full OWCP claim files or other sensitive documents in web forms; we will collect those securely at your visit.
Information collected automatically
Like most websites, the Site may automatically collect technical information such as your IP address, browser type, device type, pages visited, referring website, and the date and time of your visit, using cookies and similar technologies (see Section 6).
Calls and text messages
If you call or text the numbers on this Site, we receive your phone number and the content of your message. Calls may be recorded for quality and training purposes where permitted by law; if so, you will be notified at the start of the call.
4. How we use information
We use website information to respond to your appointment request or inquiry; to schedule and confirm appointments and send appointment-related reminders; to answer questions about our services and the OWCP process; to operate, secure and improve the Site; to understand how visitors use the Site; and to comply with legal obligations. We do not sell your personal information.
5. How we share information
We share information only as needed to operate the Site and our practice: with service providers that process data on our behalf, including our web form provider (JotForm), website hosting, email, phone/text messaging and analytics providers, each of which is permitted to use the information only to provide services to us; with the U.S. Department of Labor's Office of Workers' Compensation Programs and your employing agency as part of your workers' compensation claim once you are a patient, as described in our Notice of Privacy Practices; and as required by law, such as in response to a subpoena, court order or lawful government request, or to protect the rights, safety or property of our patients, staff or others.
6. Cookies, analytics and advertising
The Site may use cookies and similar technologies, including Google Analytics, to measure traffic and understand how visitors use the Site, and may use advertising platforms such as Google Ads to measure the effectiveness of our advertising. These tools may set cookies or collect device identifiers. You can control cookies through your browser settings, and you can opt out of Google Analytics using Google's browser add-on. We configure analytics and advertising tools so that they do not receive the contents of appointment request forms or other health information.
7. Text messaging (SMS)
By providing your mobile number and requesting an appointment, you agree that we may contact you by text message about scheduling and appointment-related matters. Message and data rates may apply. Message frequency varies. Reply STOP to opt out at any time or HELP for help. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes.
8. Data security and retention
We use reasonable administrative, technical and physical safeguards designed to protect the information we collect. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security. We retain website inquiry information for as long as needed to respond to you and for our legitimate business and legal purposes; information that becomes part of your medical record is retained as required by New York law and our record-retention policies.
9. Your choices and rights
You may request access to, correction of, or deletion of the personal information you submitted through the Site by contacting us using the details in Section 1, subject to our legal obligations to retain certain records. Rights regarding your medical record are described in our Notice of Privacy Practices. If you are a resident of a state with a consumer privacy law that applies to us, you may have additional rights, which you may exercise by contacting us.
10. Children
The Site is intended for adults. We do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided us information through the Site, please contact us and we will delete it.
11. Third-party links
The Site links to third-party websites, including U.S. Department of Labor and ECOMP resources and Google Maps. We are not responsible for the privacy practices or content of those sites.
12. Changes to this policy
We may update this Privacy Policy from time to time. The effective date at the top of the page shows when it was last revised. Continued use of the Site after changes are posted means you accept the revised policy.
